Life Care Plan Coordinator

Position Description

The Life Care Plan Coordinator is responsible for record maintenance, client advocacy, and inter-agency communications. The Life Care Plan Coordinator will be assigned a specified case load and represent Arcare as a corporate guardian, life care manager, and advocate for individuals with disabilities who are enrolled in the Arcare Life Care Plan Program. This is a full-time position. Hours are generally M-F 8:00 a.m.-4:30 p.m. though they may vary.

Required Skills

Excellent listening skills
Excellent written language skills
Ability to communicate with, have patience with and compassion for individuals with disabilities
Ability to work within established budgets
Organizational skills for maintaining client files and attention to detail
Ability to prioritize tasks
Ability to use email, texts, Word documents, Excel spreadsheets, Zoom.

Position Requirements

A bachelor’s degree in social work or related field is required.
Employment experience in a related field may be considered if the educational requirements are not met.
Valid driver’s license and good driving record.
Weekend on-call shifts may be required.
